Some young men have normal erectile and ejaculatory functions before marriage. Another type is reflex erection, which occurs when stimulation received by the glans penis is transmitted via nerves to the reflex erection center located in the sacral spinal cord, and then the penis erects through the autonomic nervous system. In a normal erectile response, these two different pathways complement each other, resulting in an erection. The latter is more effective, but the former also plays a significant role.
From the perspective of erectile dysfunction, some cases are caused by disorders of the vascular nerves responsible for erection, endocrine imbalances, systemic diseases, or the use of certain medications that inhibit erectile function. Others experience erectile dysfunction due to emotional factors, such as fear or anxiety about sexual activity, worry or doubt about whether penis size will affect their partner's sexual satisfaction, or excessive physical fatigue, high mood, family misfortunes, or marital discord. The former is called organic erectile dysfunction, and the latter is called psychogenic erectile dysfunction. Therefore, while the brain's thinking can promote sexual arousal and erection, it can also have the opposite effect, inhibiting sexual function and causing erectile dysfunction.
Because the treatments for organic and psychogenic erectile dysfunction are completely different, it is essential to differentiate between these two types of erectile dysfunction before treatment. The presence or absence of nocturnal erections is the most effective "litmus test" for differentiation. During sleep, the cerebral cortex enters an inhibited state, completely losing control over the lower centers. Psychogenic factors in erectile dysfunction cannot play a role, allowing the lower centers to receive even the slightest stimulus and induce penile erection. Therefore, the presence of nocturnal erections indicates that erectile function is normal; the inability to achieve an erection is due to psychological inhibition, and should not be considered a problem with the reproductive organs. Treatment should first identify and eliminate the cause of psychological inhibition. Usually, after psychological counseling by a doctor to relieve anxieties, the problem can be resolved. Some people believe that a childhood habit of masturbation inevitably affects their sexual function, leading to constant anxiety and fear of failure. This psychological state can cause psychogenic erectile dysfunction. After a doctor's analysis and explanation, they understand that masturbation does not directly cause erectile dysfunction, relieving their anxieties and restoring confidence in their sexual function. Thus, normal erections can be restored without medication. However, some individuals, due to prolonged periods of unsuccessful intercourse, have developed further anxiety and created a negative conditioned reflex, experiencing erectile dysfunction simply by thinking about intercourse. In such cases, medication and sensate focus training under a doctor's guidance are necessary to resolve the issue.
